Under the supervision of the Public Health Officer, nurses will be responsible for managing the referral process for elderly patients with non-communicable diseases (NCDs) from Primary Health Care Centers (PHCCs) to hospitals, ensuring that they receive appropriate treatment and care. Below are the key responsibilities for the role:<\/p>\n
Responsibilities:<\/h5>\n\n- \n
Coordination and Monitoring<\/strong>:
\nNurses will be responsible for coordinating between the PHCCs and approved hospitals to ensure elderly patients receive timely and appropriate medical care. The nurse will work with the Field Medical Coordinator to manage referrals and ensure patients are receiving adequate treatment.<\/p>\n<\/li>\n- \n
Patient Care Coordination<\/strong>:
\nNurses will ensure that referrals are promptly made from PHCCs to the hospital for further evaluation and treatment. They must ensure that patients are aware of their treatment plans and that all procedures are carried out smoothly.<\/p>\n<\/li>\n- \n
Effective Communication<\/strong>:
\nEffective communication skills are critical, as nurses must coordinate with patients, healthcare providers at PHCCs, and hospitals. They must ensure proper flow of information and continuity of care for patients.<\/p>\n<\/li>\n- \n
Follow-Up and Monitoring<\/strong>:
\nNurses will conduct regular follow-ups with elderly patients to ensure they adhere to their prescribed treatments, attend specialist appointments, and receive continuous care for NCDs.<\/p>\n<\/li>\n- \n
Comprehensive Documentation<\/strong>:
\nNurses will ensure that all patient interactions, medical histories, and treatment plans are thoroughly documented. Proper documentation is necessary to ensure continuity of care and effective coordination between healthcare facilities.<\/p>\n<\/li>\n<\/ol>\nSkills and Competencies:<\/h5>\n\n- Minimum of 2 years of relevant nursing experience, preferably in NGOs or the humanitarian sector.<\/li>\n

- Fluent communication skills in both Arabic and English.<\/li>\n
- Ability to work collaboratively within a multidisciplinary team.<\/li>\n
